As of today, the FORM-Q017 must accompany every luminaire or lamp that is shipped to our laboratory by carrier, courier or by the customer himself. A copy of the form is attached to this notice but it can also be found in the “Forms” folder of our web site (www.spectralux.ca).

From now on, it will be impossible to start a photometric test if the information requested in this form is not present in our computer data base.

It is important that you filled all the fields of this form in order to provide you with a better service. You can fax or e-mail this form prior to your shipping or you can send it with the luminaire or lamp to be tested.
